YES Experience Reports
The YES Family Advisory Committee wants to hear stories from parents about how the YES System of Care is working well for families, or how it can improve. This is not a complaint, and anyone interested in filing an official complaint is encouraged to submit a complaint here.
Please use the YES Experience Report form below to submit your Youth Empowerment Services stories, whether they are positive or negative. They will be reviewed by the YES Family Advisory Committee to help identify barriers in the current system and for possible inclusion in a QMIA report. Your story will not be shared in the report and you will not be identified unless you indicate on the form that you are both OK with someone following up on your story and OK with it being shared. You will always be contacted before your story is shared and identifying information (name, etc.) will be removed. (Please see the note on disclosure of identifying information below) You are welcome to submit the form anonymously, but please note that it cannot be submitted without your e-mail address and we cannot contact you for additional support or follow-up if you do not provide an e-mail address or phone number.
